All enrollment and LIC forms are required, and each must include original (“wet”) signatures. Please have your child’s physician complete Section B of the LIC 701 form.
Once everything is complete, please return all forms to Orkidz Preschool in person, along with the signed last page of the Parent Handbook.
Missing or incomplete forms may delay enrollment.
